My Approach
Each patient receives baseline concierge care and we add services as needed.

7 days a week patient moitoring, caregiver support + communication, medication management, medical care team coodination up to an hour a day
24/7 on call access
Baseline care is billed on a monthly basis.
ER visits, doctors visits, in home wellness vists, emergency in home visits are billed seperately, as needed. *Night and weekend rates apply.

In home wellness visits as needed
Selecting and training caregivers to your unique needs and preferences
Twice daily communication with caregivers + ongoing as needed support for caregivers
Intepretation of caregiver updates to ensure catching health concerns early
Attending important doctors appointments either on Facetime or in person
Comprehensive care coordination: communicating with doctors before, after or inbetween appointments to make sure every provider is informed and pertinent medical information is relayed
Medication coordination [from making sure meds get sent to the right pharmacy for quickest access to knowing the best way to administer medication with resistant patients]
