During that visit, I'll assess you and your family's needs and preferences, we'll discuss how conceirge care works and we can discuss fees.  This is our time to see if working together would be a good fit. 

Whether we decide to work together or not, I will give you guidance about best next steps. 

If we decide to work together, I’ll send you a contract to review, schedule the initial assessment and send you an invoice for the initial assessment.

During this in person initial assessment, I’ll perform a basic physical exam in person, look at current lab and medical reports, take a medical history, review at current medications + care team.

After the assessment, I will develop a plan, which will include:  obtaining and reading medical records to form a clear landscape of needs, hiring and training caregivers to your unique medical needs and personal preferences, change any doctors, if necessary.
